TASC provides needs assessments and case management services to Chicago Housing Authority (CHA) residents at the Henry Horner Homes targeted for relocation to new developments.

The TASC Approach

The initial level of TASC involvement with CHA residents is the in-home Family Needs Assessment. The assessment is completed using a strength-based approach and identifies strengths and barriers to successful lease compliance, relocation transition, and stabilization within the new residence. The assessment areas include but are not limited to:

  • Physical - basic survival skills
  • Financial - income resources and money management
  • Housing - lease compliance and/or transition and stabilization issues
  • Family - support systems and child/parental needs
  • Emotional/Psychological - mental and other behavioral health issues
  • Educational/Vocational - employability
  • General Health - excluding references to specific handicaps

The assessment can lead to the development of a service plan. The plan is used to prioritize goals and activities. Reviews and modifications are made to the plan through ongoing assessment and case management. If the family requests case management services, TASC provides those services before and after relocation to support a smooth transition.
