Winnebago County Divert Program

TASC provides screening and assessment for substance use disorders, links participants to treatment and other community-based services, coordinates care and advocates for participants, monitors and reports on progress, and supports successful completion of program requirements.

Other services include:

• Assistance addressing housing or food insecurity
• Medicaid enrollment assistance
• Support obtaining state identification
• Connection to employment opportunities
• Workforce development and job readiness resources
