Bridge to Home (B2H)

B2H is a permanent supportive housing initiative designed to help people returning from the Illinois Department of Corrections (IDOC) to St. Clair, Madison, and Sangamon counties.

The program works to break the “revolving door” cycle of incarceration and homelessness by removing reentry housing barriers and providing pathways to stability.

Participants receive TASC’s Specialized Case Management and access to a coordinated network of community services that support recovery, reintegration, and long-term independence.
