The Juvenile Justice Collaborative (JJC)

TASC provides care coordination to youth involved in the justice system. Through a trauma-informed approach, care coordinators assist with developing care plans that are family-driven, youth-guided, and culturally and linguistically responsive to youth and family needs. 

This program connects young people to services and support based on their needs, including:

  • Academic support
  • Treatment
  • Counseling or therapy (group, family, or individual)
  • Assistance with job-seeking or workforce development
  • Life skills
  • Mentoring
  • Socialization and recreational activities

TASC manages the intake and referral process for JJC and coordinates service planning, linkage, monitoring and reporting for the program and referral stakeholders.
